We work diligently to bring awareness to advance care planning, and we invite
community members to partner with us through fundraising events, educational
opportunities, volunteer positions, and more. In addition, we participate in
various community events, such as expos, conferences, and classes.

Are you interested in partnering with Light The Legacy to promote informed decision-making and positively impact end-of-life care in Central Minnesota? Become a certified advance care planning facilitator!
Facilitators meet with individuals and families to explain various end-of-life treatment options and interpret often-intimidating medical terms. In addition, they guide the conversation to important topics that may be difficult to discuss and provide emotional support, making sure the experience is comfortable and positive for everyone involved. The facilitator can play an important role in preparing the individual’s chosen healthcare agent, and will provide information and resources as needed. Finally, a facilitator assists in the completion of a health care directive, the document that outlines the health care values and desires for treatment expressed during the conversation. The advance care planning process, which is sometimes thought to be overwhelming or uncomfortable, becomes simple and straightforward with the help of a certified facilitator.
Advance care planning facilitators can also impact the larger community by providing formal or informal education opportunities or coordinating advance care planning activities. Qualified facilitators may even assist with the development and implementation of systems that support advance care planning and provide performance reviews to improve the quality of systems currently in use.
